    Please read application Instructions & Guidelines carefully before completing. Suggested donation application fee is $38.00 or $28.00 for students (with a copy of student ID).

    After filling out this form:

    2. Mail a hard copy of your form along with your check and other materials to From the Heart Productions.

    3. You are entitled to a free 15 minute consultation when you apply for any of our grants.
    All applications must be postmarked by the official grant deadline date. From the Heart requests a Producer credit and a rolling Thank You credit to all of the donors on the grant. This application, if chosen as a winner, may be published or used to educate other filmmakers by FTHP.
